Editing Incident/Hazard Reports
UQSafe is the platform used by UQ staff and students to report hazards and incidents.
Additional details may be available following the initial reporting of an incident.This guide provides instruction on how to edit an incident/hazard report.
Who does this process apply to?
Person affected - the person affected (or potentially affected) by the incident or hazard. Note: Affected persons cannot edit records unless they were the person who reported the incident/hazard or are subsiquently invited into the record.
Reported by - the person logging the report into UQSafe. Although most often the person affected, all UQ Staff and Students can report hazards and incidents via UQSafe, on behalf of someone else.
Person responsible - the supervisor of the affected person, assigned in the system to create the management action plan.
Safety team - the relevant organisational unit's safety team are required to provide support to supervisors to develop management action plans, and verify that the proposed controls are appropriate, and the target risk is proportionate.
Invited people - Any person with a UQSafe profile may be invited to a record, giving them access to edit both the report and the management plan.
Step 1: Finding records
Incident/Hazard reports can be found through multiple avenues within UQSafe depending on access levels.
My Reports
Reports you submitted, or are the affected person can be found in "My Reports". To access my reports either click the "My reported hazards/incidents" link within the I want to view tile, or within the menu in the top left corner (9 dot "waffle") and click the "My Data" button.
Manage
Reports you are the person responsible, or are invited to can be found in "Manage". To access my reports either click the "Reports assigned to me to manage" link within the I want to view tile, or within the menu in the top left corner (9 dot "waffle") and click the "Manage" button.
Visualisation links
IF you have visualisations on your dashboard the reports related to the data displayed can be accessed by clicking on the segment of data you want to view.
Hazards and Incident Reports Register
Safety teams have access to the "Hazards and Incident Reports Register" for their area. To access the Hazards and Incident Reports Register click the menu in the top left corner (9 dot "waffle") and click the "Health and safety" button, followed by the Hazards and Incident Reports button.
Step 2: Editing records
Editting
- Access to edit mode requires a user to click the "Edit" option within the commands menu (3 dots), or click edit within the record at the top of the page.
- Edit any details.
- Click one of the "update" buttons located at both the top and bottom of the page.
Restrictions
- A record which is closed or canceled cannot be editted.
- If you want to edit a closed record, the record must be re-opened. This can only be done by a safety team member.
- To re-open a record "Re-Open" option within the commands menu (3 dots), and provide the reason for doing so.
- Affected persons cannot edit records unless they were the person who reported the incident/hazard, is assigned the incident/hazard or are subsiquently invited into the record.
Principles
- Where possible, do not edit open text fields (though they can be added to), as the orginal contect may be lost.
- Where possible, let the person affected, the person who reported, and the person assigned know of any changes which could change the context of the record.
Step 3: Editing Management Plans
Editting
- Access to the Manage Page requires a user to click the "Manage" option within the commands menu (3 dots), click the "Manage" button (checklist icon), or click manage within the record at the top of the page.
- Management plans are automatically in edit mode until a record is closed.
- Edit any details.
- Click one of the "update" buttons located at both the top and bottom of the page.
