R3 - Transfer management responsibility for an incident or hazard report
Managers or supervisors of staff and students who submit an incident or hazard report in UQSafe are responsible for reviewing the details of the report and creating a management action plan to resolve the hazards.
If you believe you have been incorrectly assigned as the person responsible for creating the management action plan, you are able to transfer this responsibility to another person.
Who does this process apply to?
Person affected – the person affected (or potentially affected) by the incident or hazard.
Person responsible – the assigned person responsible for managing an incident/hazard report. When an incident or hazard is reported in UQSafe, the person responsible for creating the management action plan defaults to be the affected persons manager or supervisor.
Safety coordinators - at UQ this includes HSW Managers and Work Health and Safety Coordinators (WHSCs) who operate at the Organisational Unit level. When verifying the details of an incident or hazard report, WHSCs are required to confirm that the affected person’s supervisor (person responsible) has been correctly assigned.
When does management responsibility for an incident or hazard report need to be transferred?
If you have been incorrectly assigned as the person responsible to manage an incident or hazard report, you are able to transfer the responsibility for creating the action plan to another person.
Note: When you transfer an incident or hazard report you will have no further access to it. We recommend you invite people to view an incident report if you need to keep management responsibility.
Step 1: Select the incident/hazard report to be transferred
Open the (waffle) menu and click on 'Manage' to see the incident or hazard reports assigned to you.
Step 2: Transfer the incident/hazard report to the new person responsible
- Find the report you are transferring management responsibility from, and open the action menu.
- Select 'transfer'.
- A box will appear asking who you are transferring management responsibility to and why. Start typing the name of the person you are transferring to and click their name to select them.
- Add notes why you are transferring the report to this person, and click transfer. A detailed reason helps the person understand why you have transferred management responsibility to them and you need them to do.
- Click yes to confirm the transfer.
The report is now removed from your manage register.
The new Person Responsible will be sent a system generated notification. The email will advise that they have been nominated as the person responsible for managing the incident/hazard and creating the action plan.
