R3 FAQs - Troubleshooting and support

An incident or hazard report will be closed automatically when all associated tasks are completed by the person assigned the task. The status will change to 'closed'.
In case there are no associated tasks to be completed, the Safety Team members will need to close the report manually. 

To close a report as a Safety Team member, click on the waffle icon on the navigation bar and open the Health & Safety Module. All incident and hazard reports for your area will be displayed in the Hazard/Incident Report Form Register.

Find the relevant report, click on 'Actions' and select 'Close'.

Please remember to check on any outstanding tasks prior to closing the report because all related tasks will be closed automatically as well.

You might be unable to find the incident or hazard report for the following reasons: 

1. Draft status: the report has not been submitted and only saved as a draft. Ask the reporter to submit the report in UQSafe. The status will then change from 'draft' to 'new'.
2. Confidential report: the report might be marked as confidential, making it invisible to the Manager/Supervisor. Individuals logging sensitive reports are able to bypass the usual workflows, and prevent notifications from being sent to the local Manager/Supervisor or Safety Team member. 

From January 2025, new hazards and incidents will be reported and actioned in the new version of UQSafe.

Incidents reported prior to this date will remain in the previous UQSafe R2 version and will continue to be accessible until all historical reports have been finalised and the actions closed out. No historical data will be lost or become unavailable. Ongoing access to a view only version of the register will be maintained during the period of transition.
